NEIGHBORHOOD WATCH APPLICATION

Empower 7’s Neighborhood Watch program supports teens ages 13–18 who are experiencing family instability, involved in the child welfare system, or at risk of removal from their home. Many of the real costs of caring for a teen in crisis fall outside what state stipends or traditional services can cover. That’s where we step in—bridging the gap with practical resources, mentoring, and spiritual care that bring hope and stability.

Through Neighborhood Watch, Empower 7 connects youth and their caregivers to services offered through our Blessings and Growth programs, including emergency housing support, crisis response, mentoring (for select youth), and ongoing relational and spiritual support.

Who Can Apply

Community members, caseworkers, or caregivers may use this form to nominate or request help for a teen whose needs are not being met through existing services.

If you—or a family you know—needs help meeting an out-of-pocket expense related to a youth in care, we invite you to apply below. Together, we can help ensure that no teen falls through the cracks.

A resource family includes foster families, kinship caregivers (relatives or close family friends), and families providing support or housing for homeless or unaccompanied youth.

Application Process

  • Before you begin, please review the form and gather all required information.

  • For CPS placements, please include the youth’s identification number, child placing agency, and caseworker’s name and contact information.

  • If applying for a group of siblings, complete the form using the oldest child in care as the primary youth in care. Include information about the other siblings within the relevant questions and need description.

  • Once submitted, an Empower 7 team member will contact the applicant within 7-10 days with next steps.
